Health Forms: You can obtain the required "Personal Health And Medical Record Class 1 and Class 2" forms here: http://www.scouting.org/filestore/pdf/34414.pdf

Class 3 forms are avaliable here: http://www.scouting.org/filestore/pdf/34412.pdf

Note: The Class 1 form is for Scouts and adult participants and needs to be updated annually (by parent or self).

The Class 2 form is for Scouts and adult participants and is good for 36 months. It requires a health examination, and must be filled out by a licensed health-care practioner.

The Class 3 form is for Scouts, and adult participants over 40, for high adventure activities and has to be updated annually. It requires a health examination, and must be filled out by a licensed health-care practioner.

 

Tour Permits:

Troop Registration: Once a year, at the beginning of the calendar year, the troop must conduct a registration of each scout and adult. The following three forms are required for registration.

New Scouts and Parent Volunteers must fill out the proper forms shown below:

  • Boy Scout Registration form 28-209.pdf from http://www.scouting.org/forms/

Transfer form (Used when transferring a scout to another troop or a leader wishes to transfer):

  • Youth/Adult member form (pdf) Front side of form is for youth transfer and back side of the form is for adult transfer.

  • Medical Exam Class 3: 34412.pdf http://www.scouting.org/forms/ "All Class 3 activities require a health examination within the past 12 months by a licensed health-care practitioner. This includes youth and adult members participating in high-adventure activities, athletic competition, and world jamborees. Annually, this form is to be used by adults 40 years of age or older for all activities requiring a physical examination and applies to all Wood Badge participants/staff regardless of age."
